On 13 October 2023, the Central Drugs Standard Control Organisation (CDSCO) approved Nexcar19 (actalycabtagene autoleucel), India’s first indigenous CAR T-cell therapy, priced at approximately ₹30-40 lakh — against $400,000+ (₹3+ crore) for imported equivalents like Kymriah and Yescarta. Developed by ImmunoACT, an IIT Bombay spin-off, Nexcar19 treats relapsed or refractory B-cell leukaemia and lymphoma. The approval puts India in a small global club of countries with indigenous cell and gene therapy manufacturing.

What is CAR T-cell Therapy

CAR T-cell therapy is a cell-based gene therapy. It genetically modifies a patient's own T-cells (a type of white blood cell responsible for killing infected or abnormal cells) to recognise and attack cancer cells.

The therapy is named after Chimeric Antigen Receptors (CARs) — synthetic receptors added to the T-cells that help them bind specifically to cancer-cell surface proteins.

Why T-cells Need Help Against Cancer

Mechanism Step by Step

  1. Leukapheresis — T-cells are extracted from the patient's blood.
  2. Genetic modification — Using a viral vector (lentivirus) or CRISPR, a CAR gene is inserted so T-cells express the chimeric receptor.
  3. Expansion — CAR T-cells are multiplied in the lab over 7-14 days.
  4. Lymphodepletion — Patient receives mild chemotherapy to make room for infused cells.
  5. Infusion — CAR T-cells are injected back.
  6. Tumour kill — CAR T-cells multiply in vivo and eliminate cancer cells expressing the target.
