UPSC guide to Ayushman Bharat Digital Mission (formerly NDHM): ABHA, HPR, HFR, UHI, data privacy, federated architecture, 2024-26 developments.

India’s digital public infrastructure (DPI) — Aadhaar, UPI, CoWIN — has become a global reference model. The Ayushman Bharat Digital Mission (ABDM), launched as a pilot in six UTs in August 2020 and rolled out nationally in September 2021, extends this DPI approach to healthcare. By late 2024, over 70 crore Ayushman Bharat Health Accounts (ABHAs) had been created — one of the world’s largest digital health identity rollouts.

This guide explains the mission's architecture, privacy model and implementation status.

What is ABDM?

ABDM — formerly the National Digital Health Mission (NDHM) — aims to build a national digital health ecosystem that:

Implementing agency: National Health Authority (NHA).

Consent Architecture

ABDM uses the Data Empowerment and Protection Architecture (DEPA) — a consent-based framework:

NHA-empanelled Consent Managers as of 2024 include commercial platforms and public-sector implementations.

Federated, Not Centralised

ABDM does not store patient records centrally. Data remains with:

ABHA is an index; records are retrieved on demand with consent.

Privacy Framework

The Digital Information Security in Healthcare Act (DISHA) was drafted in 2018 but superseded by the DPDP approach.

Latest Developments (2024-26)

70+ crore ABHAs by late 2024.

U-WIN Roll-out. Universal Immunisation digital platform launched nationally 2024 — integrates with ABHA.

Insurance Claims Digitisation. IRDAI push for seamless PMJAY and health insurance claims via ABDM.

Tele-MANAS. Operational in all states by late 2024.

DPDP Implementation. DPDP Act 2023 rules being finalised; health data processing will be tightly regulated.

Health Data Exchange. HIEs (regional) being operationalised.

AB PM-JAY Integration. Beneficiary Identification System increasingly ABDM-linked.

Ayushman Vay Vandana (2024). Extends PMJAY to senior citizens — new ABHA enrolments surging.

Caste Census 2025. Union Cabinet approved caste enumeration in the next national census (April 2025); digital health coverage will be monitored with caste disaggregation.

MPI 2024. Global Multidimensional Poverty Index 2024 confirmed 415 million Indians exited multidimensional poverty between 2005-06 and 2019-21; digital health access correlates with poverty exit sustainability.

Women's Reservation Act. The Nari Shakti Vandan Adhiniyam 2023 — post-delimitation implementation; digital health design must factor in women's access constraints.

Global Gender Gap Index 2024. India ranked 129/146; ABHA adoption shows gender gap in digital health uptake.

Possible mains questions

  1. "The Ayushman Bharat Digital Mission builds on India's digital public infrastructure success. Examine its architecture." (250 words)
  2. "Health data privacy must go beyond consent to accountability. Discuss in the context of ABDM." (150 words)
  3. "Interoperability is the real test of digital health. Evaluate India's progress." (150 words)

Prelims pointers. NDHM pilot August 2020; national ABDM launch September 27, 2021; NHA implementing agency; ABHA — 14-digit ID; HPR, HFR, PHR, UHI; DEPA consent architecture; NDHB 2019 (J. Satyanarayana Committee); DPDP Act 2023; AIIMS Delhi ransomware (November 2022); U-WIN (2024).

Essay themes. "Digital health, human care"; "Records that travel with the patient"; "The privacy frontier".

ABDM's success depends not on ABHA numbers but on interoperable records, patient trust and strict privacy enforcement. Digital health is a contract between state, provider and patient — the Act's test will be the one of everyday trust.
