On 15 August 2021, from the Red Fort, the Prime Minister announced that all rice distributed through the Public Distribution System (PDS), Mid-Day Meal and ICDS would be fortified with iron, folic acid and vitamin B12 by 2024. That commitment made India the largest food fortification operation ever attempted. By 2024, 80%+ of rice distributed under PDS is fortified. At the same time, independent scientists have raised concerns about iron overload, hypervitaminosis and a fragmented approach to malnutrition.

What is Food Fortification
Food fortification is the deliberate addition of essential vitamins and minerals to widely-consumed staple foods to improve their nutritional content, whether or not those nutrients were originally present. The goal is to tackle "hidden hunger" — micronutrient deficiency that persists even when calorie intake is adequate.
FSSAI identifies five staple foods for fortification in India:
- Rice — Iron, folic acid, vitamin B12.
- Wheat flour — Iron, folic acid, B12.
- Salt — Iodine and iron (double-fortified salt).
- Edible oil — Vitamins A and D.
- Milk — Vitamins A and D.
Why Fortification is Needed
- NFHS-5 (2019-21) data — Stunting 35.5%, wasting 19.3%, underweight 32.1%.
- Anaemia — Every second woman in India; 67% of children under five.
- Global Hunger Index 2023 — India ranked 111 of 125, “serious” hunger level.
- Economic loss — Malnutrition costs India 3% of GDP (World Bank).
- Micronutrient deficiency — Iron, iodine, vitamin A, D, B12, folic acid most common.
Rice Fortification Technologies
- Dusting — Micronutrient powder sprinkled on rice (lost on washing).
- Coating — Nutrients fused onto rice surface with a protective film.
- Extrusion — Broken rice mixed with nutrients and extruded into "rice-like kernels" called FRK (Fortified Rice Kernels). Most effective and widely used.
- FRK is blended with normal rice in a 1:100 ratio.
Benefits of Fortification
- Wider coverage — Staple foods reach every household through PDS.
- Quick results — Anaemia and micronutrient deficiency respond within months.
- Cost effective — Fortifying a litre of oil costs around 10 paise; Copenhagen Consensus estimates 9 rupees of benefit per rupee spent.
- Culturally neutral — No change in food habits.
- No change to taste or texture.
- Scalable — Existing PDS and milling infrastructure can be leveraged.
- Complementary to dietary diversification, not a replacement.
Criticisms and Risks
- Fragmented approach — Malnutrition is multi-dimensional; fortification alone cannot solve calorie, protein and diversity gaps.
- Hypervitaminosis and iron overload — Iron alone does not build haemoglobin. Excess iron raises ferritin, linked to gestational diabetes.
- Infection interaction — Iron at acute infection phase may worsen TB, malaria.
- Loss of protective phytochemicals during polishing and blending.
- Anaemia and iron deficiency — Not perfectly correlated; anaemia has multiple causes.
- Subsidy cost — Fortification raises PDS cost.
- Small industry impact — Medium/large millers receive fortification subsidies; small mills struggle with Rs 3.2 crore setup costs.
- Monopoly risk — Premix supply controlled by a few firms.
